"Mad Love" was written by Lisa Greene, Brad Daymond, Alex Greggs, Rodney Price, and Dennis Shaw. It was produced by Greggs.
in 2022, Greggs revealed in an interview that the instrumental was produced in 2004, while the lyrics and vocals were recorded during the early Blackout sessions in 2006.

- While the song was recorded during Blackout, the actual song was pitched to Spears for her sixth studio album, Circus, but was rejected.
- Some fans thought the song was intended for Spears' third studio album, Britney, as her vocals sounds similar to that era.
